This assessment is designed to provide a baseline of function following onset of disability. This is completed by an occupational therapist in the home to establish the level of the individual's performance in daily activities associated with self-care, productivity, and leisure roles. Where required recommendations are made regarding services, equipment, treatment and/or other investigations required to maximize function, and/or enhance quality of life.
Short term follow up by the occupational therapist is available to insure safety in the home and community, and to introduce intervention for improving function.
The Occupational Therapist makes an evaluation of the level of required attendant care needs for the individual as a result of a motor vehicle injury. Specifically, attendant care needs for routine personal care, basic supervisory care, and complex heath care functions are identified using an Assessment of Attendant Care Needs (Form 1).
